Since about one year, we've been quite happy with the Mantis bug tracker, an Open Source trouble ticket system. My client is now going to phase out a home-grown Lotus Notes & Domino trouble ticket system (which was bloody lousy, though certainly no fault of Notes) and will be moving with quite a number of projects to Mantis.

Even though there are a number of bug tracking systems out there (Gnats, Bugzilla, etc.) I like Mantis because it is feature-rich enough and not too complex to use.
